It is recommended that all students be covered by health insurance. If you are not carried on a family health insurance policy, a comprehensive student health insurance policy is available. Frequently Asked Questions about Mandatory Student Health Insurance

MANDATORY PLAN (FAQ)
Students who are required to have health insurance will be enrolled each semester in the Mandatory Plan, which is an accident and sickness insurance policy that includes diagnosis and/or treatment of illness, injury, or medical conditions. Benefits include physician, hospital, surgical, pharmacy, behavioral health services (i.e., mental health / substance abuse), as well as legally mandated benefits.

Mandatory Plan Highlights
Following are a few policy highlights. More details about the plan benefits and limitations will be available soon.

  • Maximum Lifetime Benefits = $1 Million
  • Maximum Benefit Per Illness / Injury = $300,000
  • Maximum Annual Deductible
    • $250 per Insured Individual (for In-Network Providers)
    • $300 per Insured Individual (for Out-of-Network Providers)
    • $750 per Insured Family (for In-Network Providers)
    • $900 per Insured Family (for Out-of-Network Providers)
    • The deductible is waived for the Named Insured when treatment is rendered at the University Health Center.
  • Maximum Annual Out-of-Pocket (Stop Loss)
    • $2,500 per Insured Individual (for In-Network Providers)
    • $4,000 per Insured Individual (for Out-of-Network Providers)
    • $7,500 per Insured Family (for In-Network Providers)
    • $12,000 per Insured Family (for Out-of-Network Providers)

Mandatory Plan Premium
Premiums for individual students in the Mandatory Plan are as follows:

Fall Semester 2008: $407
Spring/Summer Semester 2009: $539


$10.00 Administration fee per semester (Fall & Spring; included in fee)

Students may also purchase health insurance coverage for their spouse and children for an additional premium. More information will be made available soon regarding dependent coverage.

There will be an option for students who can demonstrate that they have a qualifying policy through a spouse or other source to waive the statewide mandatory policy, but the details of the waiver are not known at this time. Students are urged to determine if their insurance plan considers the GSW Health Center to be "In-Network" status and that the deductible is waived if treatment is rendered at the GSW Health Center.

OPTIONAL PLAN
All GSW students who are not required to have health insurance may purchase the Optional Plan if they are (a) enrolled in six (6) or more semester hours or (b) participating in Cooperative Education Programs.

The Optional Plan is an accident and sickness insurance policy that includes diagnosis and/or treatment of illness, injury, or medical conditions. Benefits include physician, hospital, surgical, pharmacy, behavioral health services (mental health / substance abuse), as well as legally mandated benefits

    • The deductible is waived for the Named Insured when treatment is rendered at the University Health Center.

    • $4,000 / Individual for In-Network
    • $5,000 / Individual for Out-of-Network
    • $12,000 / Family for In-Network
    • $15,000 / Family for Out-of-Network

Optional Plan Premium
The Optional Plan premiums for individual students will be $1,351.00 per year. Students may also purchase health insurance coverage for their spouse and children for an additional premium. Various payment options are also available for the Optional Plan, including annual and semesterly payments.
