The Health Center is as concerned about your good health as it is with your educational development. Only with good health - both physical and mental - can you fully attain your academic potential. You and Georgia Southwestern State University together are making a substantial investment in your education. It is sensible to protect that investment through good health care practices, prevention of disabling illness whenever possible, and by obtaining the quickest and most effective treatment when an illness or accident does occur.
All students enrolled for 4 semester hours or more at Georgia Southwestern State University are eligible for services. Proof of enrollment (such as current student ID) must be shown before receiving service. University affiliated students in the English Language Institute are eligible for medical care. Emergency first-aid and referrals are available to faculty, staff, and visitors at no cost.
Your Georgia Southwestern State University medical record is confidential. Information will not be released without your written consent except for legal subpoenas and cases of some communicable diseases, which by law, must be reported to the Department of Public Health.
Request for Immunization Records/Medical Records
How do I obtain my GSW Immunization Records or Medical Records?
Immunization records and medical records must be requested in writing to the Health Center by submitting the form: CONSENT FOR AUTHORIZATION FOR USE/DISCLOSURE OF HEALTH INFORMATION. Request must include student's full name, gsw ID#, student's phone number, a complete address or fax number where the records are to be sent, signature, and appropriate fee. The consent forms can be completed at the Health Center or accessing the above link. Records may be requested by submitting the consent form in person, via email (email@example.com) or fax (229-931-2666). In order to avoid any delays, please be sure to include all names you may have used while attending GSW.
What is the charge for Immunization and/or Medical Records?
Immunization and/or Medical records are $1.00 per copy. Health Services accepts payment in the form of cash, check, or money order. Credit or Debit card transactions can be processed using the new Touchnet Payment System (see link on GSW Home Page) or may be processed through the cashier in the business office (220-931-2013).
How long does it take to process my requests for records?
Records are usually processed within 24 hours of receipt of the completed request. This time may vary during registration and graduation periods.
I need my records sent out a.s.a.p. Must I pay the records fee immediately in order for my requests to be processed?
Yes. Your request will not be processed until we receive payment. Please include form of payment with your requests to avoid any delays. If you are picking up your records from Health Services, you may indicate on your requests that you will pay at time of pick up.
Can I have someone else pick up my immunization or medical records?
You may have someone else pickup your request if you send written permission for that person to pick up your request. The person picking up your immunization records must have proper identification.
